1. The employer is not following the Guidance for Cleaning and Disinfecting to develop, follow, and maintain a plan to perform regular cleanings of surfaces to reduce the risk of people's exposure to the virus that causes COVID-19. 2. The employer asks and allows employees who are known to have tested positive for the COVID-19 virus to return to work without first following the recommended CDC quarantine, and or, isolation guidance. 3. Safe work practices such as providing resources and a work environment that promote personal hygiene are not provided for employees. For example, tissues, no-touch trash cans, hand soap, alcohol-based hand rubs containing at least 60 percent alcohol, disinfectants, and disposable towels for workers to clean their work surfaces in the areas where approximately half the staff have tested positive for COVID-19.Alleged Hazards: 4, Employees Exposed: 50
