1. The employer has not effectively developed or implemented an Infectious Disease Preparedness and Response Plan. The use of masks or face covering is not enforced; employees and managements are not wearing masks properly. Contact tracing has not been conducted with employees that has been in close contact with positive COVID-19 cases so they either could quarantine or get tested. 2.
