1. The right side kitchen oven door hinge is broken, allowing the heavy oven door to fall open. Multiple employees have been injured by the oven since the door broke on New Year's Eve Day. 2. Proper PPE, specifically gloves, are not adequately provided to employees. Employees have experienced chemical burns due to using cleaning products without wearing gloves. 3. COVID-19 guidelines are not being followed or enforced. Management personnel are not wearing masks around employees or guest. 4. AC Unit near the prep table leaks water onto and around the prep table causing a slipping hazard as well as a contamination hazard to the food being prepared in the area.Alleged Hazards: 4, Employees Exposed: 5
