1. The employer is not conducting daily surveys of changes to staff/labor health conditions. The employer purchased a temperature scanner but is not monitoring employees' daily symptoms using the device. 2. The employer is not complying with the Governor's COVID-19 mandate in regard to quarantining symptomatic employees. Multiple employees were exposed to COVID-19 after a symptomatic employee was allowed to continue going to work regardless of testing positive for COVID-19. 3. The employer did not report an accident occurring in the course of employment within 24 hours which resulted in the inpatient hospitalization of one or more employees.
