1. Failure to comply with the mandate for employees to wear face coverings. 2. Multiple employees were being allowed to continue to work while they were sick and later tested positive for COVID-19. Multiple employees working in close proximity to the COVID-19 infected co-workers were exposed to the spread of COVID-19. 3. After the employer was made aware that multiple employees tested positive for COVID-19, they did not sanitize and clean the affected areas in the building.Alleged Hazards: 3, Employees Exposed: 50
