1. The employer failed to assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, employees may be exposed to potential hazards such as when they were conducting fogging in the classrooms without PPE. 29 CFR 1910.132(d)(1)(i-iii) [Refer to 12-60-50(a), HAR] 2. Employees were not provided with effective information and training on hazardous chemicals used for fogging in their work area.
