The employer did not identify, evaluate, and control the hazards from COVID-19 in the workplace, exposing employees to health hazards. The employer failed to engage in adequate and effective infection prevention and to implement the proper control measures to protect its workers from COVID-19 by not enforcing the use of masks by employees. On 12/11/20, some employees in the office reported that they were not feeling well. They came back to work on 12/14/20. Some employees got tested on 12/16/20. Employees continued to work until 12/18/20. An employee got positive test results, then two other employees came back positive. No deep cleaning or sanitation of the office was done.
