1. The employer has not implemented an effective process for screening employees for COVID-19 symptoms. T8 CCR 3205(c)(2)(B) 2. The employer has not ensured that employees are able to maintain at least 6 feet of distance from other employees during lunch. Approximately 30 employees take their lunch break at the same time and all sit close to each other at 4 large tables. T8 CCR 3205(c)(6)(A) 3. The employer has not enforced the use of face coverings in the workplace. Supervisors routinely walk by and ignore employees who are not covering their nose with their face covering. T8 CCR 3205(c)(7)(A) 4. Employees climb into running sorting machines to service and repair them without de-energizing all power sources including the in-feeding belts. T8 CCR 3314(c)&(d) 5. The employer has not ensured that the belts are maintained in good working order, and that employees not walk on the belts. During the night shift employees are required to climb up onto the belt and walk on it in order to activate the powered belt system. T8 CCR 3328(b) 3999(f) 6. The employer does not provide foot protection to employees who are exposed to falling objects such as the handle heavy boxes they are required to handle. T8 CCR 3385(a) 7. The employer has not identified and corrected the hazard of pedestrian employees being struck by powered industrial trucks. The employer has not delineated powered industrial truck areas and pedestrian walkways, and has not trained pedestrian employees on how to safely walk across the warehouse. T8 CCR 3203(a)
