1. Employee hospitalized with Covid-19 work related and not reported to OSHA. 2. Employee was exposed to Covid-19 because there was no decontamination fluid available to clean ambulance unit after transporting Covid-19 patient. 3. No Safety Data Sheets for decontamination fluid. Fluid stored in unlabeled jugs that were prior used for washer fluid. 4. PPE that is provided is ill fitting, too small and not complete. 5. Black mold in the building at 104 E. 7yh street. 6. Building is infested with bed bugs and roaches. 7. Strong odor of sewer gas. 8. No GFCI in kitchen and bath areas. 9. Bunk areas are sprayed with pump sprayer that has a label of skull and crossbones. No other identity. 10.
