1-Temperatures are not being check for employees on a daily basis in regards to covid-19 symptoms. 2-First aid kit at the site have expired items in them. 3-When there are confirmed cases of Covid-19 in the workplace the employer does not notify other workers to monitor themselves for signs/symptoms of Covid-19. 4- Employees are required to operate the rough terrain (forklift) without having been trained and certified. 5- Employees are exposed to concrete burns on their skin as they are not provided with proper Personal Protective Equipment such as gloves and full body protective suit. 5- Boom Lift on site has exposed and tangled cables on the boom's extension arm. 6-Personal Protective Equipment such harnesses, which are used as fall protection devices, are not in good condition. Thus, exposing employees to fall hazards. 7-Employees that are required to go into confined spaces such as, but no limited to, tanks are not trained on the procedures and precautions to follow inside confined spaces. 8-Employees are required to go into confined spaces without the air quality being monitored before the enter the confined space. 9-Employees are required to go into confined spaces without filling out and completing a permit. 10-Employees working on concrete activities are not protected against concrete burns as they are not provided with proper Personal Protective Equipment such as gloves and full body suit.Alleged Hazards: 10, Employees Exposed: 5
