The employer is not following the guidelines set forth in Executive Orders 20-54 and 21-01, which include COVID-19 Preparedness Plan Requirements for Minnesota businesses, specifically: 1. The establishment does not have a written COVID-19 Preparedness Plan in place. 2. The establishment is not requiring employees to wear face coverings when working in the facility. 3. The establishment does not have procedures in place to conduct health screenings of staff to identify individuals with symptoms of or potential exposure to COVID-19. The following are in addition to the above allegations: 4. Forklifts operated within the facility are not kept in good working condition and are in states of disrepair. 5. Forklift operators drive recklessly within the facility and are not disciplined or retrained to drive safely.
