1. The employer was not following covid-19 protection policies in October (e.g., not having patients mask in home to protect staff). 2. In November, the employer had not fit tested all staff but required all staff to see covid-19 patients. 3. The fit tester was instructed to fit test all employees the following week. The director initially disagreed but relented days later. 4. The employer does not require patients or their families to wear a mask to protect employees. 5.
