1. 24 hour heightened cleaning or closure of areas within clinic which had COVID positive not followed per policy after clinic had 3 COVID positive patients in the facility. 2. An employee was allowed to come into work today presenting with COVID related symptoms, such as nausea and headache, after being exposed to a COVID positive patient last Wednesday 11/25. 3. A COVID positive patient was in the clinic last Wednesday 11/25, per company and MIOSHA policy, deep cleaning was not completed within 24 hours, nor has it been completed prior to other patients/providers utilizing the space.
