1. The workplace has no barriers between stations and does not use face shields when employees are within less than 6 ft of each other. 2. There are no masks on the floor. 3. There is no sanitizer on the floor. The only sanitizer is at the main doorway. 4. The computer kiosk is not sanitized after each separate employee use. 5. The break room does not have tables marked off to ensure six feet of separation. Marks are not on the floors anywhere to ensure proper spacing. 6. They are not doing a symptom questionnaire daily, only temperature checks. 7. The COVID-19 preparedness plan is not available to employees. 8. There are no hand washing posters in the facility. 9. There are 10 people out, 6 with positive tests and some quarantined from a staff of 79. A COVID-19 positive employee, reported a positive test result to the company on Tuesday, November 17, 2020. HR interviewed the employee for contact tracing and it was discovered the employee had 8 workplace contacts that fit the "within 6 ft for 15 minutes or more" description between Tuesday, November 10, 2020, and Friday, November 13, 2020. HR provided management the 8 person list. Management indicated if the 8 employees were sent home it would shut down the operations if they had to quarantine. The list was given back to HR and all the names were essentially crossed off with a statement that the employee was not within six feet or less for 15 minutes or more of these people.
