The employer is knowingly and willingly placing staff and residents in imminent danger of COVID-19 exposure due to failure to disclose (inform employees) fact of COVID exposure within facility. The facility is not properly sanitized in accordance with CDC and other government agencies. There is no posting of recent COVID exposure warnings. The residents are not using/wearing protective masks or washing their hands often. The facility is requiring staff to falsify safety and unit sanitizing documents so it appears that the units are being cleaned thoroughly. Employees are falsifying reports and failing to report abuse to disabled residents. There is retaliation from residents who have been directly injured and retaliation from administration if brought to their attention. The facility is misusing facts that individuals are on medication and diagnosed as disabled as a reason to ignore safety policies and procedures. Location: Within Harold Jordan Center Facility on each unit (BSU, FAU)Alleged Hazards: 1, Employees Exposed: 100
