1. Forklift operators are not certified prior to work assignment. 2. Forklifts are not inspected daily for defects. 3. Pallets of foam balls are overflowing onto the floor causing slipping hazards. 4. Employees are wearing headphones while operating forklifts. 5. The company doesn't have a vermin control program in place. 6. Broken pallets are being stored up to 25 feet high and have fallen over. 7. Trailers parked in loading docks are not being locked out and have pulled away while forklifts were inside. 8. Forklifts are being driven too fast throughout the warehouse and horns are not being used. 9. Dock plates are broken and not locking into place on the back of trailers. 10. The employer is not following the CDC guidelines for social distancing of 6 feet during lunch and break times.Alleged Hazards: 10, Employees Exposed: 50
