1. The facility in not kept in a sanitary condition in that employees are required to clean unknown chemicals that have spilled from overfilled trucks. Employee must walk through 3 inches of chemicals that have spilled to the floors and walls. 2. Forklifts are not cleaned or disinfected between drivers to protect employees from the spread of COVID. 3. Breakrooms are not cleaned or disinfected to protect employees from the spread of COVID. 4. Employees who operate forklifts are permitted to operate using Bluetooth speakers and/or headphones and are unable to hear other vehicle traffic or employees. 5. Employees who have had accidents while operating the forklifts have not been recertified prior to operating the lifts again.Alleged Hazards: 5, Employees Exposed: 1
