1. Employees are presenting COVID-19 symptoms and employer is not enforcing CDC Guidance procedures. Employees are not instructed to get tested and are permitted to stay working exposing other employees to COVID -19. and later on tested COVID-19 positive. 2. Approximately 26 employees have been tested positive for Coronavirus (COVID-19) and the facility has not been cleaned or disinfected in accordance with CDC Guidelines. 3. Employer is not enforcing the use of face mask at all time; employees are provided with personal protective equipment (PPE) such as but not limited to facemask, but are not trained to wear the mask properly in accordance with CDC Guidelines for Businesses and Employers to prevent exposure to coronavirus (COVID-19). 4. Company has not implemented risk assessment procedures for Coronavirus (COVID-19) to prevent employee exposure.Alleged Hazards: 4, Employees Exposed: 26
