1. Failure to comply with the mandate for employees to wear face coverings. Seven employees (warehouse staff, front desk staff, supervisors and owner) observed not wearing face coverings. 2. Failure to comply with social distancing measures by maintaining a minimum six-foot distance between employees in public spaces. All employees observed eating lunch in the kitchen and not adhering to social distancing requirements. 3. The owner is exhibiting signs and symptoms of COVID-19 and is continuing to work, potentially exposing employees to the contraction of COVID-19.Alleged Hazards: 3, Employees Exposed: 1
