The employer is not implementing recommended control measures to protect employees from SARS-Cov-2, the virus that causes COVID-19 illness, resulting in an unreported workplace-related fatality: 1) An employee returning from travel in Florida where high levels of community spread of SARS-Cov-2 was present, was not required to isolate prior to returning to the workplace. The employee, showed signs of symptoms related to COVID-19 but remained at work. Shortly after this exposure, a nurse co-worker contracted COVID-19 and died. 2) An educator worked in the psychiatric inpatient area had COVID-19 symptoms including coughing and was allowed to continue to work near other staff. 3) An employee who was notified on July 10, 2020 of workplace exposure to employees positive with COVID-19 during recent shifts died from COVID-19 related illness on August 5, 2020 and the employer did not notify OSHA of a workplace related COVID-19 fatality.
