1. Positive COVID employees allowed to continue working on a makeshift COVID hall with negative and positive residents. 2. They are using a separate entrance through a maintenance room but go through part of the kitchen to do so. 3. An Aide working on the COVID hall was found using the employee break room when they should not have been. 4. Limited on housekeeping so they cannot keep this place clean.Alleged Hazards: 4, Employees Exposed: 1
