There are multiple issues occurring in the Neonatal Intensive Care Unit (NICU). 1) COVID-19 positive infant within the NICU is paired in an assignment with non-COVID patients, allowing for cross-contamination within an extremely vulnerable population of critically ill, premature infants. 2) COVID-19 negative father has been allowed to visit since birth, despite direct contact with COVID-19 positive mother. He was not required to quarantine. 3) Father has been permitted to move from mother's sick unit (she was hospitalize beyond pregnancy/delivery due to COVID-19 positive status) to NICU without regard to cross-contamination despite his admission and healthcare employee witness to refusal to use proper PPE when in contact with mother. 4) COVID-19 positive mother has been allowed to visit without requiring any quarantine. She was first permitted in the unit at 10 days post onset of symptoms.Source: Osha.gov | Receipt Date: 2020-11-16
