There is an outbreak of COVID-19 among employees throughout the facility and the appropriate controls have not been implemented to prevent further spread. Specific concerns include but are not limited to: 1. Administrative and engineering controls have not been put in place in location where employees work in close proximity to one another and cannot wear masks due to extreme heat. 2. Contact tracing is not performed when employees test positive for COVID-19. 3. Employees with prolonged close contact with COVID-19 positive coworkers are not notified of their potential exposure and are not asked to quarantine. 4. Employees are not screened prior to starting their work shift to identify symptomatic employees and prevent them from entering the workplace. 5. The workplace is not being cleaned and disinfected on a regular basis or after an employee has tested positive for COVID-19.Alleged Hazards: 5, Employees Exposed: 500
