1. Employees wearing N95s have not been given a medical questionnaire, training, and fit test. 2. Employees COVID-19 positive are still working exposing others employees. 3. Employees are required to go in an out of the COVID-19 unit (skilled unit) to other parts of the facility. COVID-19 unit is not adequately blocked off from the rest of the building. 5. There is not enough PPE provided.
