1. There is mold and mildew in the Blend Room. Approximately 20 employees are exposed; some employees are experiencing sinus symptoms. 2. COVID-19 pandemic plan has not been developed and implemented for employees to include employee wellness checks, disinfecting and sanitizing, social distancing, personal protective equipment, informing employees of possible exposure, employee return to work and employee training. Employees are not required to wear face covering in the establishment. Several employees have been diagnosed with COVID-19. 3. Ceiling tiles containing asbestos are being disturbed in the Blend Room.
