1. Employees are concerned for their safety while operating the "Zamboni machine" due to the machine sliding on the ice as the brakes are being applied. The machine operator's vision is blocked by the "snow dump" while the conditioner is being dropped. 2. Employees are exposed to struck by hazards while attempting to raise the broken garage door. The door doesn't lock and could come back down on employees. The door is heavy and has a tendency to become stuck. There is shelving being held together by zip ties which has fallen before, exposing employees to a struck by hazard. 3. Employees are exposed to slip and fall hazards while attempting to move the nets on the ice due to the employer not providing the proper safety boots. 4. Employees are concerned about the conditions of the facility due to the ceiling collapsing everywhere and a crack approximately 2-3 inches wide in the wall which extends from the top to the bottom. 5. Employees are exposed to trip and fall hazards throughout the facility caused by the rubber mats lifting up in places. There is a sunken area of the floor around the ring being covered by a rubber mat, exposing employees to trip and fall hazards. 6. The employer is not protecting employees from customers during COVID-19 pandemic and is not informing employees of positive cases.Source: Osha.gov | Receipt Date: 2020-11-10
