1. Visible mold growth is present on the walls in the kitchen area, inside the restroom toilets, and around the restroom sinks, thus exposing employees to respiratory hazards. 2. A foul odor is emitting throughout the restaurant due to a clogged sewage pipe, thus exposing employees to respiratory and biological hazards. 3. The restrooms are not maintained in a sanitary condition in that there is dirt and debris accumulation on the floors and toilets, thus exposing employees to biological hazards. 4. Paint chips and black-colored dust is falling from the ceiling, thus exposing employees to respiratory hazards. 5. There is dust accumulation on the lamps in the dining room area, thus exposing employees to respiratory hazards. 6. The employer has not implemented the Centers for Disease Control and Prevention (CDC) guidelines as they relate to the Coronavirus Disease (COVID-19) into the workplace to include elements such as, but not limited to: a) Enforcing the usage of facemasks or face coverings; b) Requesting employees that are sick or experiencing COVID-19 related symptoms to stay home; and c) Ensuring social distancing is practiced by employees to maintain at least six (6) feet from each other.Source: Osha.gov | Receipt Date: 2020-12-30
