Entire Clinic Area M - F 8 AM - 6 PM Patient Care Hours M - TH 9 AM - 5 PM, Lunch 12 PM - 1:30 PM COVID 19 related hazards to workers: Employer provided a clinic assistant with a N-95 (filtering facepiece respirator) mask. Required to reuse, not fit tested, no medical waiver, was unable to obtain non-soiled or used respirators, told to wear them 5 times. Employer provided a single KN-95 mask to two administrative staff to use daily, contacting public and others, both in front and back of clinic, during and after aerosolizing procedures. Employer provided 4 masks with face shields as an alternative to using KN-95. Employer does not have a respirator protection program. No required trainings.Source: Osha.gov | Receipt Date: 2020-10-29
