1. An employee tested positive for COVID-19. After management received the employee's positive test result, other employees were notified of who the employee was. 2. A resident was recently relocated to this facility from a quarantined location. Employees were not informed that they would have a resident that may have had COVID-19 or had been in a quarantined location. The resident was in close proximity to other residents and employees prior to passing away, possibly from COVID-19. 3. Employees experiencing COVID-19 symptoms were told by management that they are not to quarantine or call in sick. 4.
