1. T8 CCR §3362(f): Clinic has cockroaches and crickets enter through a hole in the ceiling. 2. T8 CCR §5199(e)(1): Handing out a limited number of masks to employees, employees wearing same mask for up to two months. 3. T8 CCR §5199(e)(2): Always running out of sanitizer, and cleaning supplies related to COVID-19. Note: Complainant's state that they're forced to falsify the amount of COVID related supplies the clinic actually receives when billing their insurance company.
