The employer does not follow their COVID-19 preparedness plan and uniformly quarantine employees who have been exposed to COVID-19. Extension cords are damaged and not repaired in a safe condition. Chains and slings used for lifting are not inspected as required. Jigs used for lifting materials have not been proof tested, rated, and inspected on a regular basis.
