1. No covid training has been provided or documented. 2.There is no covid response plan document for staff to access. 3.There is no designated covid supervisor. 4. Health screenings for staff are not being routinely performed. 5. Regular cleaning of all surfaces is not being performed. 6. There is no consistent enforcement of proper mask wearing by management. 7. There are more scheduled appointments currently than there were pre-covid. There is no limit on the number of persons allowed in the office. 8. Testing is being performed without masks on equipment that cannot be thoroughly disinfected. 9. On 2 known occasions, patients have called in saying they tested positive shortly (1 day) after being seen. The office was not closed, no additional cleaning was performed and the affected staff were not informed of potential exposure, nor were any affected patients informed of potential exposure. Employees are concerned that there may have been other exposures that they were not informed of by management. 10. Following exposure, staff are expected to continue working and are not told to isolate. 11. Many staff are concerned for their health and that of their families, yet those concerns are dismissed by management who informs employees that because they are wearing masks, they are not at risk.
