1. The doctor does not wear a mask unless patients come in. He interacts with staff without a mask. 2. Patients that are possibly COVID positive or confirmed positive are still brought into the office. No procedures have been developed to address this issue. 3. Employees aren't provided face shields. 4. Cleaning is inadequate. Supplies not appropriate for COVID, facility not cleaned regularly, especially when possible COVID patients present and use the facilities. 5.
