1. Employees not trained or certified are allowed to operate forklifts. Employees drive forklifts recklessly and near misses have occurred. 2. Employees who perform welding are exposed to welding fumes and other particulates. Ventilation is inadequate. 3. Employees are exposed to struck-by hazards due to damaged wood beams holding up part of the facility's structure. 4. Bathrooms are not maintained in a clean and sanitary condition. 5. Employees are exposed to COVID-19. The employer does not ensure that employees wear masks continuously.
