1) No measures were taken to protect employees from exposure to COVID-19 after a resident tested positive for COVID-19. 2) Employees who are showing signs of COVID-19 are allowed to continue to work. 3) Resident rooms are unclean and residents have to sit in feces on their toilet. 4) Medication is left out and residents who aren't prescribed the medication have access to it. 5) 3rd shift is allowed to sleep on the clock.Alleged Hazards: 5, Employees Exposed: 1
