There is an outbreak in the building: at least 6 employees have tested positive for the virus or have been diagnosed with the infection; 2. The employer has not informed employees who had, or could have had, contact with affected employees; 3. Since the outbreak started, the facility has not been cleaned and sanitized; 4.
