The following items are alleged: 1. Annual Truck crane Inspections are performed and non- certified inspectors who are not trained, nor certified to perform this task. An inspection approval sticker is applied to the crane. 2. A job-made man basket elevates employees on the front forks of the fork truck. This is basket was built for waste materials not to elevate workers, nor approved for use as a man basket by the fork truck manufacturer. 3. (No) COVID l9 Hazard Risk Assessment was performed. Assess job or work task, the work place layout, nor hazards and places where PPE is currently used including the device and reason for use. The form used should check the type of hazards present within each section and (one) of (four) appropriate hazard risk levels, then determine the controls to protect against COVID 19 (Engineering, Administrative, and work practice controls against contracting COVID 19). Make documents accessible to employees once completed, signed and dated, keep the documents in a binder easily accessible to employees. 4. The employer does not have an COVID infectious Disease Control Plan. 5. The employer has not provided employees training on What is COVID 19 ?; How to protect themselves from contracting the COVID-19 virus?; nor J D Newman's Infectious Disease Control Plan? 6. The employer does not provide employees mask, hand sanitizer, nor require social distancing to protect employees from contracting the COVID-19 virus.Alleged Hazards: 6, Employees Exposed: 50
