Beginning on or about March 25, 2020, 1) The employer did not implement the written respiratory protection for employees who provide direct Covid-19 patient care. Specifically, the employer did not: a) Provide initial fit testing for respirators when the brand, model and/or size respirator changed. b) Provide training on respirator donning and doffing. c) Provide training or enforcement of User Seal Checks. d) Provide end of service life training. e) Provide respirators that were NIOSH approved or approved through other approval agencies. 2) The employer did not provide training on donning, doffing, proper care, maintenance and useful life for personal protective equipment such as, but not limited to, gowns and face shields. Employees are required to reuse this personal protective equipment. 3) The employer did not record illness of employees who contracted Covid-19 and experienced lost work days and medical treatment on the OSHA 300 Log. 4) Employees were not provided training on the hazards of cleaning chemicals used to decontaminate their personal protective equipment such as, but not limited to, gowns and face shields.
