1)Employees in Booker 3, the Medical/Surgeries Care Unit, were not informed of the COVID presence in the unit in order for them to take protective measures. Several employees tested positive for COVID 19 after working with COVID positive patients. There was no notification as to potential exposure; no training on safety, no signage and no PPE provided to prevent worker exposure even after management knew that the patients on Booker 3 were positive for COVID-19.Alleged Hazards: 1, Employees Exposed: 100
