1. The establishment is not practicing social distancing at all. The establishment has a full house constantly and refuses to turn people away. 2. Personal protective equipment is not available for employee use. 3. Tables, counter tops, and other surfaces are not being disinfected. 4. There are new rules in place now and the employer doesn't even have a list for names of contacts for tracing as required. 5. Workers have been affected but the employer doesn't even follow proper quarantine procedures to ensure employee safety at his establishment.
