1. An inmate tested positive for the covid-19 virus and the other inmates who had contact with the inmate were not isolated. 2. Employees are not provided with sufficient PPE: they have N95 masks only, no gowns or face shields are provided. 3. Employees are not wearing masks when they within 6 feet from inmates. 4. Employeee temperatures are not taken or recorded. 5.
