1. The employer has had an outbreak of the COVID-19, and employees are concerned that not enough steps to mitigate the outbreak are being taken and that infection control practices are inadequate. 2. The employer is not taking appropriate measures to protect employees and patients from COVID-19. Employees are being required to work even if they test positive for COVID-19, and patients are leaving their rooms after testing positive for COVID-19. 3. Employees are concerned that the facility is not performing to required standards when not under regulatory agency inspections. 4. The employer is limiting the PPE supply and usage, and requiring PPE reusing of gowns, masks, and face shields. 5. Employees are concerned about a lack of vitals taking instruments.
