1. From March 18, 2020 to April 8, 2020 employees of Aggregate Industries were being transported from the office building, employee parking lot area, to the upper work site in a crew cab truck. There would be 4 to 6 employees in the cab of the truck, not following the social distancing guidelines of maintaining 6 feet of distance to prevent the spread of COVID-19. The truck would then return the employees to the office building at the end of the shift. The employees would be in the vehicle for approximately 5 minutes per trip. 2. The company has still not provided cleaning supplies for any of the mobile equipment, the mobile equipment is operated by multiple operators. 3. The management of the company implemented the 6 foot social distancing requirements for the upstairs office area, which is the management area, on or about March 18, 2020, but did not implement or enforce the 6 foot social distancing requirements for the workforce.
