1. Employees are exposed to possible infection to COVID-19 in that no one is mandated to wear proper personal protection equipment (PPE) while working, contrary to the OSH ACT of 1970, General Duty Clause 5(a)(1). 2. Employees are exposed to possible infection to COVID-19 in that no isolation of exposed staff was implemented when approximately eight (8) positive cases throughout the facility have shown infection, contrary to the OSH ACT of 1970, General Duty Clause 5(a)(1). 3. Employer has not reported three (3) COVID-19 hospitalizations within 24 hours of said hospitalization, contraction of COVID-19 was said to have occurred at the worksite, contrary to 29 CFR 1904. 39(a).Alleged Hazards: 3, Employees Exposed: 30
