1. Staff worked closely with physician who was feeling unwell on October 21, 2020. Estimated 50 patients seen by this physician on that date. Physician tested positive for COVID-19 in the ensuing days and cancelled office hours for October 26th through 30th. 2. Management did not notify staff who worked closely with infected physician or direct them to quarantine. 3. No guidance provided to staff on what to do following an infection within the workplace: how or when to quarantine, office cleaning/disinfection procedures, patient notifications. 4. Similar scenario happened six weeks prior when two members of front desk contracted COVID-19: no immediate notifications to staff, no attempt to clean/disinfect the office was made.Source: Osha.gov | Receipt Date: 2020-10-27
