1.) An employee who had been symptomatic with COVID-19 symptoms, i.e. coughing excessively, in the workplace tested positive for COVID-19. Employees who had potential exposure to the positive employee have not been required to quarantine. 2.) Following the positive COVID-19 test, the workplace, specifically the areas where the employee was located and equipment that the employee was using, were not properly sanitized and disinfected. 3.) Prior to the employee testing positive, the employer did not require and enforce the use of face masks, social distancing procedures, and frequent disinfection of commonly touched items for the prevention of, and protection from, COVID-19 in the workplace.Alleged Hazards: 3, Employees Exposed: 50
