1. The employer has hired minors and they are using manufacturing tools such as, heat (upstairs for hospital gowns production Iron Machine) and saw machines (used to cut PVC). They are exposed to their associated hazards. 2. Forklift/Order Picker operators are not provided with fall protection training and are exposed to heights of at least 2 stories when loading and unloading storage racks in the production area and warehouse. 3. Failure to comply with the mandate for employees to wear face coverings. Office workers, including the owners, are not wearing face coverings and are not enforcing use of face coverings. 4. Failure to comply with social distancing measures by maintaining a minimum six-foot distance between employees in public spaces. Social distancing is not being practiced or enforced in the breakroom, during breaks, or lunches. 5. The employer is hiding reports of employees contracting COVID-19 by firing them and not informing possibly affected employees.
