1. N95 fit testing denied. N95 fit testing denied for approximately 120 employees including Villa employees & contract employees through YONA Healthcare Services (housekeeping & laundry contract services) and Energize Rehab (Physical therapy, occupational therapy & speech therapy contract services) 2. Failure to notify: Three employees have been identified as COVID positive & staff were not notified. On Aug 1, 2020 a Business office employee received a positive COVID test. Staff was not notified as required (within 12 hours) as no notifications were provided on the orders of Regional Director Of Operations & Executive Vice President. On Sept 8, 2020 two additional employees received positive COVID results. This was the administrator & director of nursing.
