1. Face coverings are not being enforced to be worn, or worn improperly. 2. Hand sanitizer is out in the restrooms. 3. Cleaning/sanitizing supplies have run out and are not available for cleaning work areas/equipment. 4. Social distancing is not followed - specifically for a meeting in which 20+ employees were gathered in the breakroom for a conference call with HR. 5. After an employee tested positive, there was no indication the work area was shut down for cleaning.Alleged Hazards: 5, Employees Exposed: 100
