1) The individuals conducting the COVID-19 Swab Tests at Rennes Health and Rehab Center - East are not wearing the appropriate PPE. An individual performing the COVID-19 Swab Tests last week was only wearing a surgical mask and a flipped up face shield. A helper with the COVID-19 testing was wearing street clothes. 2) The Rennes Group has not implemented any safe guards for Rennes Health and Rehab Center - West CNAs working shifts at the Rennes Health and Rehab Center - East facility to minimize the potential of re-introducing COVID-19 at the Rennes Health and Rehab Center - West facility. 3) Rennes Health and Rehab Center - West staff are being tested for COVID-19 at the Rennes Health and Rehab Center - East facility. Rennes Health and Rehab Center - East is currently experiencing a COVID-19 outbreak. The Rennes Group has not implemented procedures to minimize the potential of Rennes Health and Rehab Center - West staff being infected with COVID-19 at the Rennes Health and Rehab Center - East facility during testing.Alleged Hazards: 3, Employees Exposed: 200
